feat(covers): концепт обложки по смыслу статьи (LLM)

Вместо общей метафоры по ключевому слову — haiku придумывает конкретную
визуальную сцену под конкретную статью (generateVisualConcept). Это убирает
'обложки не по теме'. При ошибке LLM — откат на getVisualMetaphor.
